What's in the supplier scorecard template
The main sheet is a scoring matrix: up to 5 suppliers across the top, 5 weighted criteria down the side — on-time delivery, quality/defect rate, cost competitiveness, responsiveness, and compliance/documentation, each with a default weight you can adjust to your own priorities.
You score each supplier 1-5 on each criterion, and the weighted total for each supplier calculates automatically, so you can compare suppliers side by side on one screen instead of averaging scores by hand.
A second sheet, Review History, logs each supplier's weighted score by review date over time, so you can track whether a supplier is trending up or down across quarters.
How the rating tier is calculated
In the Review History sheet, each logged weighted score automatically maps to a tier: 4.00 and above is Preferred, 3.00-3.99 is Approved, 2.00-2.99 is Conditional, and below 2.00 is At risk. You don't type the tier — it's driven by the score you enter.
That means the tier updates the moment you log a new review, giving procurement a consistent, repeatable label to use in supplier conversations instead of a subjective call each time.
Supplier scorecard vs vendor list vs generic scorecard
This template is specifically for weighted performance evaluation — scoring suppliers on delivery, quality, cost and responsiveness to produce a rating tier. If you just need a directory of vendor contacts with no scoring, the vendor list template is the simpler fit.
It's also distinct from a generic scorecard template built for tracking things like sports or team scores — this one is built around procurement criteria and weighted formulas specific to supplier evaluation.
