Free Lean Six Sigma Templates (Excel)

This free Lean Six Sigma template links three tools in one workbook: a Project Charter, a SIPOC process map, and an Action Tracker that flags overdue improvement actions automatically. Competing template hubs ship these as three separate, unconnected files. Works in Excel and Google Sheets.

What's inside the free Lean Six Sigma templates

Project Charter holds the problem statement, business case, goal statement, project scope, team lead and team members - the same information a Six Sigma project sponsor needs to approve before work starts.

SIPOC is a 10-row process map: Suppliers, Inputs, Process step, Outputs, Customers - one row per process step, with a summary that counts how many steps you've mapped so far.

Action Tracker logs up to 40 improvement actions with owner, due date and status; the Flag column reads Late the moment a due date passes and the action isn't marked Done, and the summary counts how many actions are currently late.

One connected workbook instead of three separate downloads

Smartsheet and similar hubs offer a Project Charter template, a SIPOC-style process template, and an action or implementation-plan template - but as three separate .xlsx files with no shared data between them. This template puts all three in one workbook, so a team working a Lean Six Sigma project keeps everything in one place instead of tracking three downloads.

If you only need a single dedicated SIPOC file (not the charter or action tracker), a standalone SIPOC template is the better, lighter-weight fit.

How the Action Tracker's Late flag works

The Flag column checks Status first: mark an action Done and it always reads On track, no matter the date. Otherwise it compares the due date to today - past due and not Done reads Late.

This is the same logic used across this workbook's action-tracking, so once you're used to it here it carries over to any other TableTemplates file with a Late/Overdue flag.

Who this template is for

Lean Six Sigma teams and green/black belts running a process-improvement project who want the charter, SIPOC map and action list connected in one file rather than juggling three separate downloads.

It's a working project file, not a statistics package - for control charts, Cp/Cpk or other process-capability math, pair it with dedicated Six Sigma software or an add-in.

How to use it

  1. On Project Charter, fill in the problem statement, business case, goal, scope, team lead and members.
  2. On SIPOC, map your process step by step - the summary counts how many steps you've mapped.
  3. On Action Tracker, log every improvement action with an owner, due date and status.
  4. Check Action Tracker's Flag column and summary count for anything that's gone Late.

Frequently asked questions

How is this different from a standalone SIPOC template?

This workbook bundles a SIPOC map alongside a Project Charter and an Action Tracker. If you only need the SIPOC tool on its own, a dedicated standalone SIPOC template is the lighter-weight option.

Does this include control charts, Pareto charts, or Cp/Cpk calculations?

No. It uses COUNTIF-based rollups for tracking, not standard deviation or process-capability statistics. For control charts or capability analysis, use dedicated Six Sigma software or an Excel add-in.

How does the Action Tracker decide an action is Late?

It checks Status first: Done always reads On track. Otherwise, if the due date is before today, it reads Late; if not, it reads On track.

Can I use this in Google Sheets?

Yes. Upload the downloaded file to Google Drive, then open it and choose File > Save as Google Sheets. All three tools and their formulas keep working.

How many SIPOC steps and actions does it support?

Up to 10 process steps on SIPOC and up to 40 actions on Action Tracker. Add rows and extend the formulas if a project needs more.

What's the usage license?

Personal use or use within one business. It's not meant to be resold or redistributed as a template product.
