Free Safety Audit Checklist Template (Excel)

This free checklist lets you log safety items by zone, mark each Compliant, Non-compliant or N/A, and get an automatic compliance score (N/A items are excluded). Non-compliant items get a corrective-action tracker with a due date, and anything overdue is flagged automatically. Works in Excel and Google Sheets.

A real spreadsheet, not a PDF or an app upsell

Two of the five top results for this search are PDF checklists or push you toward a paid inspection app (SafetyMint, SafetyCulture) rather than giving you an actual working spreadsheet. CIToolkit's version is a static checklist with no scoring at all.

This is a plain Excel and Google Sheets workbook: type your items in, mark compliance, and the score and overdue flags calculate themselves. No app account, no PDF you have to retype into something else.

It's built as a recurring-audit log, not a single-use form - so the same workbook works for your next audit of the same site, or a different zone, without starting over.

How the compliance score and overdue flag work

The compliance score is compliant items divided by compliant plus non-compliant items - N/A items are excluded entirely from that calculation, so they never drag the score down or artificially inflate it.

For any item marked Non-compliant, fill in the corrective action, who's responsible, and a due date. The Overdue flag only appears when all three are true: the item is Non-compliant, a due date is entered, and today's date is past it.

Compliant and N/A rows are never flagged overdue, even if you accidentally type an old date into the due-date column - the flag checks the compliance status first.

Who this template is for

Facilities managers, EHS coordinators and small-business owners running their own recurring workplace safety walkthroughs across zones like the warehouse, production floor, office and loading dock.

This is a self-assessment and tracking tool - it doesn't replace a certified safety inspector, an insurance carrier's audit, or your jurisdiction's official OSHA-equivalent compliance requirements.

How to use it

  1. List checklist items by zone and mark each Compliant, Non-compliant or N/A.
  2. For Non-compliant items, add the corrective action, responsible person and due date.
  3. Check the summary for your compliance score and any corrective actions flagged overdue.

Quality & Audit Pack

This checklist scores compliance per zone and flags one corrective action overdue at a time, but there's no severity scoring or aging across findings, and no way to schedule your next audit. The paid Quality & Audit Pack ($49) adds a 100-row NC/CAPA Register that scores severity, root cause and automatic aging in days across every finding, plus a 30-audit Audit Plan & Schedule workbook with automatic Planned/Overdue/Done status. Both build in Excel and Google Sheets, every formula tested to the cent.

Frequently asked questions

How is the compliance score calculated?

Compliant items divided by compliant plus non-compliant items. N/A items are excluded entirely, so they never affect the score either way.

Can I use this in Google Sheets?

Yes. Upload the downloaded file to Google Drive, then open it and choose File > Save as Google Sheets. The score and overdue flag keep working.

Does an old due date on a Compliant item ever trigger the overdue flag?

No. The overdue flag only checks Non-compliant rows - a Compliant or N/A item is never flagged, regardless of what's typed in the due-date column.

Can I track more than one site or location in the same file?

Yes - add a site or location note directly in the checklist item text, or duplicate the sheet per site if you want them fully separated. Zone is a dropdown for area within a site, not a separate site field.

Is this a certified safety audit or an official OSHA form?

No. This is a self-assessment and tracking tool for your own recurring audits - it doesn't replace a certified safety inspector or your jurisdiction's official compliance requirements.

What's the usage license?
