Free Vendor Scorecard Template (Excel)
What's in the vendor scorecard
The Vendor Scorecard sheet holds up to 80 vendors, one row each: quality, delivery, price and service scores from 1 to 5, plus a weight for each of the four criteria.
A weighted score calculates automatically for every vendor, and status - Preferred, Approved, or Watch - is set automatically from that score: 4.00 or higher is Preferred, 3.00 to 3.99 is Approved, below 3.00 is Watch.
The summary below the table counts how many vendors land in each status and shows the average weighted score across your whole list, so you can see your supplier base at a glance.
More than a generic download
Smartsheet offers a generic vendor scorecard .xlsx, but it's a bare template with no automatic weighting or status logic. ProjectManager's free download is a lead magnet for their project software, and the construction-blog and SaaS-blog results in this space mostly explain the concept without giving you a working file.
This template calculates the weighted score and status for you automatically once you set your weights and score each vendor - no manual math, and no need to build the IF-statement logic yourself.
It's built for generic procurement scoring (quality, delivery, price, service) across any industry, not tied to one vertical.
Who this scorecard is for
Procurement teams and small businesses evaluating suppliers on standard criteria - quality, delivery, price, service - across a growing vendor list.
For freight and carrier-specific KPIs (on-time %, damage rate, cost per mile) instead of generic vendor criteria, see the Carrier Scorecard template.
How to use it
- Enter quality, delivery, price and service scores (1-5) for each vendor.
- Enter the same four weights (quality, delivery, price, service) in every row you fill in.
- Weighted score and status calculate automatically; check the summary for how many vendors are Preferred, Approved or Watch.

Frequently asked questions
How is the Preferred / Approved / Watch status set?
It's set automatically from the weighted score: 4.00 or higher is Preferred, 3.00 to 3.99 is Approved, and below 3.00 is Watch.
How should I set the weights?
Enter the same four weights (quality, delivery, price, service) in every row - set them to add up to 1 so the weighted score stays on the same 1-5 scale as your inputs.
Is this the same as a carrier or freight scorecard?
No. This scorecard is for generic vendors and suppliers. For freight and carrier-specific KPIs (on-time %, damage rate, cost/mile), see the Carrier Scorecard template instead.
Can I use this in Google Sheets?
Yes. Upload the downloaded file to Google Drive, then open it and choose File > Save as Google Sheets. The scoring and status formulas keep working.
How many vendors can I track?
Up to 80 vendors on the Vendor Scorecard sheet, with a summary counting Preferred, Approved and Watch vendors plus the average score.
What's the usage license?
Personal or single-company use. It's not meant to be resold or redistributed as a template product.
